Tiny Travelers
Ages 0-3 years | $250/year
- 2 exams per year (exams include toothbrush prophy)

Frequent Flyers
Ages 3-13 years | $400/year
- 2 cleanings per year
- 2 periodic exams per year
- 2 fluoride treatments per year
- All necessary x-rays
- 1 limited (emergency) exam per year

Membership Plan: Rules & Conditions
- Fee must be paid up front and renewed on an annual basis.
- 1 year Membership starts from the day of the signed contract and fee completion.
- Fee is non-refundable and non-transferable.
- Patients with traditional dental insurance are not eligible for this membership plan.
- All normal financial policies apply, including fees associated with cancellations with less than 48 hours notice and no-shows.
- Membership plan is ONLY for services at Destination Pediatric Dentistry and does not apply for any other dental offices or other specialist providers we may refer your child to.
- Services only apply to normal working hours and does not apply to after hours emergency visits.
- Treatment discount is only applicable when full payment is received on the day of treatment.
